SURGICAL REHABILITATION OF SEQUELAE OF PANFACIAL FRACTURE

Abstract

OBJECTIVE: To report a case of rehabilitation of panfacial fracture sequelae.

METHODS: Follow-up from admission to hospital discharge and return.

THEORETICAL BACKGROUND: Panfacial fractures involve the frontal bone, the middle third and the mandible, or at least two of these regions. These injuries present a challenge in management to avoid sequelae. Sequelae after maxillofacial fractures are frequent and can affect quality of life.

RESULTS: A 24-year-old male patient was involved in a motorcycle accident with a panfacial fracture. Due to maxillary and nasal comminution, a closed reduction was chosen. Later the patient presented oroantral and oronasal fistulas, loss of right maxillary projection and laterorrhinia. Rehabilitation was planned with fistula closure (buccinator muscle flap), nasal (rhinoseptoplasty and use of costochondral graft) and maxillary (iliac crest graft) correction. The patient presented adequate fistula closure, gain of maxillary projection to the right and attenuation of laterorrhynia.

CONCLUSIONS: Correction of facial sequelae is a challenge, especially in trauma involving multiple fractures. Multidisciplinary treatment is necessary.
