PRE-ECLAMPSIA LIKE SYNDROME INDUCED BY SEVERE SARS-COV-2 INFECTION AT A MAJOR MATERNITY FOR COVID-19 CARE IN BRAZIL: A CASE SERIES

Abstract

Objective: To evaluate the incidence of clinical, laboratory, and ultrasound changes related to pre-eclampsia (PE) to differentiate it from PE-like associated with COVID-19. Method: Cross-sectional study analyzing the medical records of pregnant women admitted to Hospital Júlia Kubitschek, from March 2020 to March 2021, with GA > 20 weeks, confirmed Covid-19, blood pressure and renal changes. Theoretical framework: Severe SARS-CoV2 infection causes laboratory changes similar to PE and its severity criteria. Results: After analyzing 74 medical records, three patients who evolved with a condition similar to PE were selected. The impact mechanism of infection in critically ill patients shares the pathophysiological mechanism with PE, causing similar signs, even in the absence of defective trophoblastic invasion, which is corroborated by the normalization of parameters with the clinical improvement of COVID-19. Conclusion: Studies suggest that one way of differentiating between PE and PE-like syndrome would be through the sFlt-1/PlGF ratio of the IPAUt, such parameters are not always available in services in Brazil, which makes the diagnosis difficult, increasing the possibility of iatrogenic conduct.
